• Payor Name
• Payment amount for each case
• Docket number if there is more than one child support order
• Last 4 digits of your social security number, 
• Court case number
• FIPS number for the issuing County (Woodford County is 1720300)
• All checks should be made payable to the Illinois State Disbursement Unit
