Immunization Clinics
Woodford County Health Department Immunization Clinic
By appointment only, please call 309-467-3064 to make an appointment.
For children to receive immunizations at our clinic, you must provide a copy of their immunization records. For adults, please bring records if you have them available. If you do not have records, please contact our office for assistance.
If a client is under 18 years of age, a parent/legal guardian must sign the paperwork. An adult must be present for the injections on clients under 18.
If you need a copy of immunization records, they may requested by calling our office or you may request online at IDPH VaxVerify. If you received the immunizations at another facility, you will need to contact that health care provider to access those records. The Woodford County Health Department may not have records from another health care provider unless you previously shared them with us. Please note:
- Parent/Guardian must bring past immunization records with child.
- Parent/Guardian must accompany minor child in order to sign permit form.
- Those receiving vaccines will be required to wait in lobby for 10 minutes after receiving vaccine.
